After cleaning and drying your nipples, choose the correct size (Medium for areola ≤ 45 mm, Large for larger areolas). Insert the silver cup into your nursing bra so the cup rests over the nipple. You can wear them between feedings to allow the nipple skin to heal. They are designed to work passively rather than during active feeding.
